Default How to stop iTunes from making duplicates for my songs?

iTunes keeps adding duplicates for a lot of my songs. I realize that all of the duplicates are songs that I've added to playlists which I've made in iTunes.
A song such as Whatever You Like by T.I. will be added as
Artist: T.I. and Song: "Whatever You Like" and the other duplicate will be Artist: T.I. and Song: "T.I. - Whatever You Like"
or another example...the song will be Miss Murder by AFI.
One copy will be Artist: AFI & Song: "Miss Murder" and the other will be Artist: AFI & Song: "AFI - Miss Murder"
so all in all the duplicate song will be titled with the artist and a dash and then the song name...
How do I fix this??

ps - the iTunes "View Duplicates" feature is not of much value it doesn't work well...I don't understand why iTunes needs to add duplicates of 1 file in the first place...
